29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The employer is failing to protect employees from fall hazards. This was most recently documented on or about October 17, 2018 and October 29, 2018 at the residential construction jobsite located at 522 Iowa Ave., in Holton, Kansas. An employee was fatally injured when the all-terrain fork truck overturned. Employees engaged in re-roofing activities were lifted in/working from a work platform attachment on the fork truck and were exposed to fall hazards of approximately 27 feet. Employees were not provided with training to recognize, evaluate and control fall hazards while working at height in the work platform.
