29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: 15 WEST GRANBY ROAD, GRANBY, CT: On or about August 28, 2018, employees were using electrical equipment that did not have a continuous path to ground: A) An orange extension cord used to energize equipment being used in the attic was missing a ground pin. B) A Leviton 3-Way Power tap with only two wires was plugged into the extension cord noted above in order to energize work lamps and an air scrubber. The air scrubber was equipped with a 3-prong plug. C) A Dri-Eaz F284 DefendAir HEPA 500 Air Scrubber with a 3-prong plug was plugged into an ungrounded Leviton 698-W 15-Amp/125-Volt grounding triple outlet adaptor.
