29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems. 15 WEST GRANBY ROAD, GRANBY, CT: On or about August 28, 2018, employees were exposed to fall hazards while working in the attic of a house. The employees were removing fiberglass insulation from between ceiling joists. There was limited plywood for employees to stand or kneel on and no other fall protection provided. An employee fell between ceiling joists and landed on the floor of the master bedroom below. She succumbed to her injuries.
