29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about 07-17-2018, located at Norfolk Naval Shipyard; Code 106 , Portsmouth, VA 23702, Condition: An employee, while previously working onboard SSBN 742 on 07-16-2018, cleaning sea salt from the starboard side injection pump inlet went to the hospital and was admitted on 07-17-2018. The employer did not report the work-related inpatient hospitalization to OSHA until 08-16-2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
