29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: The employer is failing to protect employees by failing to instruct employees in the recognition and avoidance of unsafe conditions such as, but not limited to, crushing, struck-by and falling hazards. This was most recently documented on August 15, 2018, at a worksite located at 916 S. Thompson Avenue, Sedalia, Missouri. An employee was fatally injured and two others nearly injured when a concrete masonry silo fell on top of the employee while the employees were attempting to set the silo. The employer did not adequately train the employees in the steps necessary to safely set up and level the masonry silo.
