29 CFR 1910.217(b)(8)(iii): Mechanical power press control(s) did not incorporate a type of drive motor starter that would disconnect the drive motor from the power source in event of control voltage or power source failure and require operation of the motor. 125 Gloucester Ave, Gloucester, MA; Basement Glass Shop Employees working on mechanical power presses in the Glass shop were exposed to amputation hazards from a motor starter that would not prevent from accidental restart of the machine. This includes, but is not limited to the following instances: a) On or about August 1, 2018, the Moly Press Shop ID 24101 was operated without having a starter that prevented the motor from restarting in the event of a control voltage or power source failure. b) On or about September 6, 2018, the Moly Hole Press Shop ID 24103 was operated without having a starter that prevented the motor from restarting in the event of a control voltage or power source failure. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19.(d).
