29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A. Fiber Baler: On, or about, July 9, 2018, an employee who worked as a Baler Operator for three weeks was not trained regarding lockout procedures to be used when inserted bale wire. The employee had received general training, but had not been trained regarding when to lock the equipment out or how to lock the equipment out. B. Conveyor 81A: On, or about, July 10, 2018, an employee accessed the screener section for daily cleaning. The machine was not locked out at the main disconnect because a section of the machine was still running. The employee affixed a lock onto one of the three shaft motor disconnects, leaving the other two unlocked. When a second employee arrived, a group hasp was not used. The employees did not appear to have a working knowledge of the fundamental mechanics of applying lockout devices.
