29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or about July 10, 2018, at the Coker II Unit, the employer failed to implement safe work practices to protect employees from the hazards associated with the work area while engaged in unwiring a motor operated valve (MOV). An employee was burned by condensate/steam that was released from an uncapped flange in the immediate work area.
