29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-in hazards by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) ContractOne, Inc. at 105 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer did not ensure that each employee in an excavation was protected from injury in that employees were working in the north trench which was approximately 54 feet long and approximately 8 feet deep, with near vertical walls in type C soil, installing a water line without a trenchbox or shoring, and without adequate sloping or benching. On June 14, 2018, the trench collapsed, resulting in the fatality of one employee. This condition exposed employees to cave-in and struck-by hazards; (b) ContractOne, Inc. at 107 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer did not ensure that each employee in an excavation was protected from injury in that employees were working in the south trench which was approximately 60 feet long and approximately 8 feet deep, with near vertical walls in type C soil, installing a water line without a trenchbox or shoring, and without adequate sloping or benching. This condition exposed employees to cave-in and struck-by hazards.
