29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of actual excavation: (a) ContractOne, Inc. at 105 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer requested the location of underground utilities installations in October of 2017 to perform sewer line work, but did not request the location of underground utility installations at any time in 2018 prior excavating the north trench. This new excavation was approximately 54 feet long from the residence to the street and approximately 8 feet deep. This condition exposed employees to electrical and burn hazards. b) ContractOne, Inc. at 107 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer requested the location of underground utilities installations in October of 2017 to perform sewer line work, but did not request the location of underground utility installations at any time in 2018 prior excavating the south trench. This new excavation was approximately 60 feet long from the residence to the street and approximately 8 feet deep. This condition exposed employees to electrical and burn hazards. Abatement note: State of Colorado underground utility installation requests are valid for only 30 days from the date of the response.
