29 CFR 1910.146(i)(5): Communicates with authorized entrants as necessary to monitor entrant status and to alert entrants of the need to evacuate the space under paragraph (i)(6) of this section; a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. At the time of the entry into the permit-required confined space an attendant had not been stationed to monitor the entrant's status, to alert the entrant of the need to evacuate the space, or to summons rescue.
