29 CFR 1910.146(i)(4): Remains outside the permit space during entry operations until relieved by another attendant; NOTE: When the employer's permit entry program allows attendant entry for rescue, attendants may enter a permit space to attempt a rescue if they have been trained and equipped for rescue operations as required by paragraph (k)(1) of this section and if they have been relieved as required by paragraph (i)(4) of this section. a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. At the time that the entrant was discovered inside tanker number 7031 a coworker entered the permit-required confined space without first ensuring an attendant was on-site.
