29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about 5/22/2018, at the maintenance department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. A fan (Model No. and/or Serial No. not available) lacked a ground prong on its attachment plug, exposing an employee to an electrical hazard. b. On or about 5/23/2018, at the welding area of the designer department, CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. A fan (Model No. and/or Serial No. not available) lacked a ground prong on its attachment plug, exposing an employee to an electrical hazard. c. On or about 5/23/2018, at the welding area of the designer department, CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. A fan (i.e. Maxx Air) was being energized by means of a 2-wire extension cord, making its path to ground not permanent and continuous. d. On or about 5/23/2018, at the welding area of the designer department, CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. An extension cord lacked a ground prong on its attachment plug, exposing an employee to an electrical hazard.
