29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) EMCOR Government Services, Pentagon: On or about 04/05/2018, the employer failed to provide the proper hardware to block the exhaust fan from rotating as a result an employee's right ring finger tip was caught between the belt and pulley. The pulley rotated after the rag used to secure the energy dislodged. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
