Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) when the operator had been involved in an accident or near-miss incident to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely. The employer is failing to ensure that all employees operating powered industrial trucks have received adequate refresher training and are evaluated as outlined in this paragraph of the standard. This most recently occurred on April 11, 2018 on the job site located at 10405 J St., Omaha, NE. Employees have not been evaluated/re-trained after they were involved in an incident in which another employee was seriously injured.
