29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) 300 Langston and 322 Ward Flexo Folder Gluer(s) (FFG): On April 11, 2018 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to, pneumatic, in that the written procedures did not include specific steps for isolating, securing and verifying that pneumatic energy was released. b) 300 Langston Flexo Folder Gluer (FFG): On April 11, 2018 and at times prior, the employer exposed employees to potential hazardous energy sources such as, but not limited to, electric and pneumatic, in that the written procedures incorrectly instructed the reader to "See Figure 4" for an electrical lockout location photo, when "Figure 4" did not exist".
