29 CFR 1910.146(d)(9): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue: (a) On or about 02/23/2018 - Steel Dust Recycling, LLC, 13209 Highway 96, Millport, Alabama, Kiln #3, employees were required to enter a permit-required confined space to clean out built up materials on the interior surface exposing them to potential atmospheric hazards, including, but not limited to, hazards related to guniting and welding and procedures to ensure emergency services were provided had not been developed and implemented.
