29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about 11/1/2017, throughout the facility, workers were exposed to struck-by, caught-in, and amputation hazards where the employer failed to insure that specific procedures provided all required information for the control of hazardous energy of equipment including but not limited to the Raw grinding area line by performing a periodic inspection of the written procedures. Deficiencies in the written procedures included but were not limited to: a. intended use (application) of the procedures was not specified, i.e. cleaning, clearing, or maintenance tasks; b. magnitude of energy sources were not identified (voltage, P.S.I.) c. potential energy sources and means to isolate them were not identified. d. procedures did not indicate steps to take to verify the effectiveness of the lockout/tagout.
