29 CFR 1910.269(l)(4)(i): The employer did not ensure that when an employee uses rubber insulating gloves as insulation from energized parts (under paragraph (l)(3)(iii)(A) of this section), the employee shall also use rubber insulating sleeves: (a). Transformer on Utility Pole 0258 FTL - The employer failed to ensure that employees also use rubber insulating sleeves when using rubber insulating gloves as insulation to energized parts in close proximity. An employee received an approximate 7 kV electric shock to his upper forearm due to contact with an ungrounded and energized transformer casing while performing secondary line work, on or about 08/24/2017. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
