29 CFR 1910.269(l)(11): Non-current-carrying metal parts of equipment or devices, such as transformer cases and circuit-breaker housings, were not treated as energized at the highest voltage to which these parts are exposed, unless the employer inspects the installation and determines that these parts are grounded before employees begin performing the work. (a). Transformer on Utility Pole 0258 FTL - The employer failed to treat the transformer casing as energized before employees began work. The employer failed to inspect the transformer ground wire to determine whether it was connected before work began. An employee received an approximate 7 kV electric shock due to contact with an ungrounded and energized transformer casing while performing secondary line work, on or about 08/24/2017. There is no abatement certification or documentation required for this item.
