29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that were related to the safety of the work to be performed was not determined before work on or near the lines or equipment was started. Such characteristics and conditions include, but not limited to 1910.269(a)(4)(i) through 1910.269(a)(4)(viii): (a). Transformer on Utility Pole 0258 FTL - The employer failed to determine that the transformer ground wire was disconnected before work began. An employee received an approximate 7 kV electric shock due to contact with an ungrounded and energized transformer casing while performing secondary line work, on or about 08/24/2017. There is no abatement certification or documentation required for this item.
