29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process This violation was most recently observed on or about August 3, 2017, in the VPS #2 process unit when the employer failed to ensure the Process Safety Information (PSI) such as Piping & Instruments Diagrams (P&ID) were updated and accurate. There was a discrepancy on the diagram depicting the process water piping connected to the Atmospheric Condensers EX001 and EX005.
