29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a) Koch Foods of Gainesville, LLC - An employee operating a pallet jack was exposed to noise levels of 89.2 dBA which exceeded the extended workshift hours noise action level of 84.2 dBA. The exposure was derived from a sample taken over 514 minutes. A noise monitoring program for employees operating the pallet jack was not implemented, on or about 8/2/17. b) Koch Foods of Gainesville, LLC - An employee working at the dumping station of line #1 was exposed to noise levels of 91 dBA which exceeded the extended workshift hours noise action level of 84.2 dBA. The exposure was derived from a sample taken over 519 minutes. A noise monitoring program for employees working at the dumping station was not implemented, on or about 8/2/17. c) Koch Foods of Gainesville, LLC - An employee working at the dumping station of line #1 was exposed to noise levels of 89.5 dBA which exceeded the extended workshift hours noise action level of 84.2 dBA. The exposure was derived from a sample taken over 451 minutes. A noise monitoring program for employees working at the dumping station was not implemented, on or about 8/2/17.
