29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. On or about August 1, 2017, employees were exposed to struck-by, caught-in, and amputation hazards when there were no specific placement or removal steps for equipment such as, but not limited to, conveyors as described in the procedure titled Lockout-Tagout Procedure for Conveyors Systems and GOH Jervis Web. a. The procedure for conveyors does not indicate when the exceptions apply or when employees are to lock out at the electrical panel instead of at a motor disconnect. b. When employees lock out multiple conveyors in a conveyor system, there are no specific instructions telling the employees the order that locks are to be applied or the order that locks are to be removed. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
