29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On June 19, 2017, employees were required to access the work area of the building by utilizing an incomplete stairway with floor holes on the second floor landing, thereby exposing employees to fall hazards greater than 6 feet through the floor hole to the level below. VH Masonry Construction Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(4)(i), which was contained in OSHA inspection number 1134628, citation number 1, item number 1 and was affirmed as a final order on June 15, 2016, with respect to a workplace located at 5 N. Oakley Boulevard, Chicago, IL 6012.
