29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Plant - On or about 05/25/17 and at times prior thereto employees conducted maintenance and repairs of equipment including (but not limited to) extruders, winders, unfolding machines, air compressors, grinders, re-pelletizers, blenders, and mixers as described in the document Amcor P-1 Equipment List of date 07/18/17, and additional equipment located in Plant 2 and Plant 3, which are subjected to lockout/tagout. The employer did not train the authorized employees in relation to a) each equipment and the specific associated lockout/tagout procedure, b) the types of energies and the specific procedural steps for shutting down, isolating, blocking and securing the equipment to control hazardous energy, c) the specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and d) the specific requirements for testing the equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. Amcor, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA Inspection number 1055170, citation number 1, item number 3, and was affirmed as a final order on August 19, 2015, with respect to a workplace located at 192 Quality Drive, Thomson, GA 30824. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
