29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used: Rocky Mountain Prestress, 1302 First Avenue, Greeley, CO: On and prior to May 3rd, 2017, the employer did not ensure that employees had been trained by a competent person in the erection and use of the fall protection system at this site. Employees erected a self-retractable lifeline (SRL) system that allowed the lanyards to extend to 30 feet long. The use of this system allowed employees to free fall more than 6 feet, and to come into contact with the lower level 19 feet below. An employee used a 30 foot long SRL connected to a tripod anchored to the horizontal precast beam at Elevation #209. As the employee walked horizontally between column lines 7.85 and 6.3.6, he fell 19 feet, and came into contact with Elevation #190 below. The employee's SRL was extended out approximately 30 feet at the time of the fall. This condition exposed employees to a fall hazard of 19 feet.
