29 CFR 1926.502(d)(16)(iii): When stopping a fall, personal fall arrest systems were not rigged such that employees could not free fall more than 6 feet nor contact any lower level: (a) Rocky Mountain Prestress, 1302 First Avenue, Greeley, CO: On and prior to May 3rd, 2017, the employer did not ensure that the personal fall arrest systems were rigged to prevent a free fall of more than 6 feet, and to prevent contact with the lower level. An employee was walking/working on a horizontal precast beam on Elevation #209, and was connected to a 30 foot long self-retractable lifeline (SRL). The SRL was anchored to a tripod set up on this precast beam between column lines 7.8.5 and 6.3.5. As the employee walked horizontally approximately 30 feet, he fell 19 feet, and came into contact with the landing on Elevation #190 below. The employee's SRL was extended out approximately 30 feet at the time of the fall. This condition exposed the employee to a fall hazard of 19 feet.
