29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a- 670 Prospect Avenue, Lakewood NJ - Company employees were exposed to fall hazards, in excess of 23 feet to the ground surface below, while performing siding installation from a pump jack scaffold that was without bracing installed 4 feet above the brace to be passed, was missing end stops on both ends of the walking/working surface and was not supplied with a screen that ran the entire length of the scaffold's active walking/working surface. On the day of the worksite inspection, all of the worksite company employees had not received scaffold hazard recognition and minimization training by a qualified person. Condition noted on or about 05/03/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
