Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about December 13, 2017, at the above located workplace, the employer failed to ensure that operators and maintenance personnel implemented hazardous energy control measures prior to engaging in servicing and maintenance activities on the label machine. Employees were required to bypass a machine guard to clear jams and perform other servicing/maintenance tasks without first locking out the equipment. Employees were thereby exposed to caught-in hazards from moving and rotating parts. b) On or about December 13, 2017, at the above located workplace, the employer failed to ensure that operators and maintenance personnel implemented hazardous energy control measures prior to engaging in servicing and maintenance activities on the case sealer machine. Employees were required to clear jams and perform other servicing/maintenance tasks without first locking out the equipment. Employees were thereby exposed to caught-in hazards from moving and rotating parts. As a result of not shutting down and turning the equipment off, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application, [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing/maintenance hazard. The Gilster-Mary Lee Corporation was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1112978, citation number 1, item number 2 and was affirmed as a final order on March 3, 2016, with respect to a workplace located at 705 N Sparta Street, Steeleville, IL 62288. The Gilster-Mary Lee Corporation was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1094889, citation number 1, item number 3 and was affirmed as a final order on March 3, 2016, with respect to a workplace located at 111 Industrial Drive, Chester, IL 62233.
