29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. (a) Risser Grain, LLC - The employer did not record the August 15, 2016 amputation incident on the OSHA 300 log or its equivalent, on or about November 4, 2016. Abatement Certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
