OSH Act of 1970 Section 5(a)(l): The Employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards while cleaning hoppers: a) Motors and appliances: At the converter baghouse at rotary valve module 6 east, disconnecting means were not readily apparent and were not legibly marked. On or around August 4, 2016, an employee's hand was stuck in the rotary valve and it took approximately 45 minutes to de-energize the machine and release the employee.
