29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Railtech Boutet, Inc. - Napoleon, Ohio: On or about July 16, 2016, the employer failed to develop and implement an energy control program that includes training, written procedures, and audits to protect employees who repaired the paddles of the B packing mixer. Employees were exposed to the unexpected startup of the mixer's rotating parts while working to replace the mixer's paddles during a time the machine was not locked out. b. Railtech Boutet, Inc. - Napoleon, Ohio: On or about July 18, 2016, the employer failed to develop and implement an energy control program that includes training, written procedures, and audits to protect employees who maintained the packing mixers. Employees were exposed to the unexpected startup of the mixer's rotating parts while cleaning it during a time the machine was not locked out. c. Railtech Boutet, Inc. - Napoleon, Ohio: On or about July 19, 2016, the employer failed to develop and implement an energy control program that includes training, written procedures, and audits to protect employees who maintained the paste machine. Employees were exposed to the unexpected startup of the paste machine's rotating auger while cleaning it during a time it was not locked out. d.
