29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: location: podium level, perimeter of building on or about: 7/14/16 a) Two employees were tied off to a horizontal lifeline. Each end of the lifeline was anchored to rebar. The anchorage was not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. The employees were exposed to a fall of 12'9". location: podium level, perimeter of building on or about: 7/14/16 b) An employee was tied off to a vertical lifeline that was anchored to rebar. The anchorage was not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two. The employee was exposed to a fall of 12'9". NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
