29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On/around June 7, 2016 at the Tyson Foods facility in Hope, Arkansas, on the wall outside the machine room of the 2nd Process refrigeration, the employee were exposed live electircal parts openings in equipment were not effectively closed. a) Knockout plug approximately one inch in diameter was not installed in an electrical junction box b) Conduit fitting was missing the cover with live electrical wires inside. c) Electrical conduit in the Second Processing machinery room not sealed, exposing wire to possible damage.
