29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. On June 7, 2016 and at times prior thereto at the Tyson Foods facility in Hope, Arkansas, the employer did not review operating procedure SOP 6.2P to assure it reflect the current practice when: a) The procedure refers the user to process diagram(s) that are no longer being utilized to represent the process. b) Instruments referenced in the procedure do not appear on the P&ID that the user is directed to refer to.
