29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Milark Industries, Inc., located at 20 Baird Parkway, Mansfield, OH 44903: On or about May 9, 2016, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the turn table robotic welding cells # 100058, # 100059, and # 100071. Employees did not control multiple hazardous electrical energy sources before employees conducted activities such as, but not limited to, cleaning nozzles, changing welding tips, and adjusting welds. The OTC DR-4000 Weld Robots, Almega EX-V6 Weld Robots, and turn tables of the robotic welding cells were not isolated from electrical energy sources prior to engaging in these servicing and maintenance activities, which exposed employees to struck-by and crushing hazards.
