29 CFR 1910.266(f)(3)(xii): Transparent material that may create a hazard for the operator, such as but not limited to, cracked, broken or scratched safety glass, was not replaced: The employer is failing to ensure equipment operators are protected from struck-by hazards. Most recently, on or about 04/05/2016, at the end of County Road 4410 in Boss, MO 65440 (Logging Site), employees were exposed to the hazard of being struck by protruding objects where the employer failed to replace transparent material on both lower doors of a "Massey Ferguson" 50C front-end loader.
