29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about March 29, 2016, during the installation of a water line offset near 6684 Heisley Road, Mentor, Ohio, the employer did not ensure employees working inside a trench, with varying depth of approximately 4.8 feet to approximately 8 feet deep and near 90 degree vertical walls, was protected from cave-ins by an adequately designed protection system. The soil was classified as; Cohesive, type B.
