OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, exposing employees to amputation hazards. a). On or about March 16, 2016 employees operating the NRM 4-1/2" Cold fed rubber extrusion machines are exposed to amputation hazards because the process the employee uses to attach the hose end of one reel to the beginning hose of the next reel for continuous feed into the machine, created a condition where an employee's thumb was entangled in the thin Kevlar cord used to tie off the two ends, resulting in amputations of the thumb.
