29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) Near dock extension: On or about March 4, 2016, and at times prior, workers were exposed to contact with high voltage transformer equipment where a chain link enclosure access was not locked to prevent unauthorized entry. b) Rail mill: On or about March 4, 2016, and at times prior, workers were exposed to electrical hazards beside a walkway due to the open, Inlet Carriage 1 panel door. c) Rail mill: On or about March 4, 2016, and at times prior, workers were exposed to electrical hazards at the walkway behind LSB 1 and 2 due to energized feeder wires being capped with wire nuts without a protective enclosure. d) Rail mill: On or about March 4, 2016, and at times prior, workers were exposed to contact with more than 400 volts at the stacking bed motor control center where a panel access cover was missing from the main power supply enclosure. e)a) South rail dock crane: On or about March 4, 2016, and at times prior, crane wiring and equipment did not comply with Subpart S per 1910.179(g)(1)(i), in that workers were exposed to injuries related to electrical fires, shock, and electrocution where the crane cab heater cord was repaired with electrical tape and wire nuts. Abatement Note: Employer completed abatement during inspection.
