29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. In the warehouse area, machine guarding was not provided at the point of operation on equipment such as the S&S slotter scorer and the Langston 2 color press, exposing employees to struck-by hazards, on or about 1/11/16. Avatar Packaging, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1056142, citation number 1, item number 5 and was affirmed as a final order on 8/18/15, with respect to a workplace located at 5110 West Idlewild Ave, Tampa, FL 33634.
