29 CFR 1910.28(d)(7): Guardrails were not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, as required, of 1- x 4-inch lumber or equivalent, and toeboards, were not installed at all open sides on all scaffolds more than 10 feet above the ground or floor. a) Sound Stage Studio D- On December 10, 2015, the employer failed to ensure that all tubular welded frame scaffolding that was at a height of ten feet or more above the ground or floor, such as Spotlight 1, Spotlight 2, Spotlight 3 and Northeast and Southeast Camera Stations, were equipped with guardrails. b) Sound Stage Studio D- On December 10, 2015, the employer failed to ensure that all tubular welded frame scaffolding that was at a height of ten feet or more above the ground or floor, such as Spotlight 1, Spotlight 2, Spotlight 3 and Northeast and Southeast Camera Stations, was equipped with midrails. c) Sound Stage Studio D- On December 10, 2015, the employer failed to ensure that all tubular welded frame scaffolding that was at a height of ten feet or more above the ground or floor, such as Spotlight 1, Spotlight 2, Spotlight 3 and Northeast and Southeast Camera Stations, was equipped with toeboards on all open sides.
