29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: The employer is failing to protect employee(s) from an amputation hazard in the Syringe Line area of the facility. This was most recently documented at 1329 West Highway 6, Holdrege Nebraska when energy was not released or shut down while an employee was attempted to clear a jam utilizing a jog button. This occurred on the JC Metro injection molding machine that produces syringe plungers. The Becton Dickinson & CO. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(2), which was contained in OSHA inspection number 1021609, citation number 1, item number 1 and was affirmed as a final order on July 9, 2015, with respect to a workplace at 150 S. 1st Ave. Broken Bow, NE 68822.
