Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or about September 3, 2015, at the Hydrodeoxygenator (HDO) Unit, the employer failed to implement a safe work practice which prevented employees from servicing a valve without first verifying that the locking out of a valve upstream was adequate to prevent the release of highly hazardous chemicals (hydrogen gas).
