29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Campsite - On or prior to August 25, 2015, the employer did not provide adequate electric service to, and did not regard the manufacturer's labeling placed on the boxes' inside covers, on the campsite boxes. The campsite boxes are manufactured by Milbank, Model #U5000 Series, and are NEMA 3R (rainproof) rated. Each campsite box contained one 50 amp double-pole circuit, one 30-amp, single-pole circuit, and one 20 amp/120 volt duplex receptacle circuit, that were all supplied by one single-pole 30-amp breaker in the field boxes installed ahead of the campsite boxes. The campsite box can draw a maximum of 100 amps of service that is only being supplied by one single-pole 30-amp breaker at the field box, therefore causing the single-pole 30-amp breaker at the field box to be overloaded, which then results in the repetitive tripping of this single-pole 30-amp breaker at the field box. The undersized and inappropriately wired service exposed employees to potential hazards of arc flash from the constant resetting of circuit breaker until it faults. b) Campsite - On or about August 25, 2015 and December 4, 2015, covers were not properly fixed and secured to the field boxes creating unused openings, exposing live parts, throughout the campsite in the 120 Volt, 30 amp breakers field boxes, including but not limited to, the Panel 300 A field box which exposed employees to electrical hazards.
