29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: Jobsite located at 158 Cronin Avenue, Forestville, Connecticut: On, or about, May 5, 2015, employees were directed to work inside of a circuit breaker tank enclosure to replace bushings. The entry was made as a non-permit confined space entry once the atmosphere was tested and the interrupter mechanism was lockout out. The interior configuration of the tank including component obstructions and the slope of the floor as well as the presence of oil on all the exposed surfaces was not evaluated and addressed as potential hazards. The employer did not document the basis for determining that all hazards in the space had been eliminated before entering under a non-permit classification.
