29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: On or about 4/20/2015 at 2220 Stephens Street, Tucker, GA; where employees performing cleaning, servicing, and maintenance operations of machines including but not limited to the Mattison grinder were not trained to control potential energy sources (electrical, pneumatic, hydraulic) exposing them to amputation, electrical, caught-in, & struck-by related injuries where locks were not applied to all energy sources at all times during cleaning, servicing, and maintenance operations.
