29 CFR 1910.119(m)(1): The employer did not investigate each incident which resulted in, or could reasonably have resulted in a catastrophic release of highly hazardous chemical in the workplace: a. On or about March 3, 2015, the employer had not investigated ammonia releases from the 13,000 pound anhydrous ammonia refrigeration system, a covered process, as required under Process Safety Management (PSM) for Highly Hazardous Chemicals (HHC).
