29 CFR 1910.119(d)(3)(i): The employer did not compile information pertaining to the equipment in the process a. On or about March 3, 2015, the ventilation for the non-electrically classified ammonia engine room location had not been designed and installed in accordance with ANSI/IIAR-2, paragraph 13.3 requirements for ventilation capacity, actuation, inlet source, discharge, and control requirements to exhaust potential accumulations of ammonia refrigerants in the event of a leak in order to minimize human exposure and flammable concentrations. b. On or about March 3, 2015, the employer did not maintain documentation for pressure vessels of the ammonia refrigeration system as required in the American National Standards Institute/International Institute of Ammonia Refrigeration, ANSI/IIAR-2 Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems and in the Provisions of the ASME Boiler and Pressure Vessel Code Rules, Section VIII, Division 1, in that there were no Manufacturer's Data Reports, Form U-1's, for the High Pressure Receiver tanks, Recirculator vessels, oil pots, or evaporator surge tanks, and the High Pressure Receivers, negative 45 degree Recirculator, and negative 10 degree Recirculator had no manufacturers nameplates. c. On or about March 3, 2015, the pressure relief valves (PVR) protecting the pressure vessels and compressors of the ammonia refrigeration system did not have proper documentation in that the employer did not maintain the Form U-1 for each PRV, and the PVR installation dates had not been marked or recorded as required by the Provisions of the ASME Boiler and Pressure Vessel Code Rules, Section VIII, Division 1 and the American National Standards Institute/International Institute of Ammonia Refrigeration, ANSI/IIAR-2 Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems. d. On or about March 3, 2015, in the 13,000 pound anhydrous ammonia refrigeration system, the pressure relief valves (PRV) installed on all compressors and pressure vessels had not been sized by a qualified engineer, and the pressure relief valve to discharge piping and manifolds had not been sized properly in accordance with section 11 of the American National Standards Institute/International Institute of Ammonia Refrigeration, ANSI/IIAR-2 Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems.
